Our Nursing Home Neglect Attorneys inspected government records and discovered Wingate at South Hadley Rehab and Skilled Nursing committed the following offense:

Failed to Have a program that investigates, controls and keeps infection from spreading.

Based on observation, record review and staff interview, the facility failed to follow proper infection control practices for the prevention and spread of infection for 1 sampled resident (#13), in a total sample of 23 residents.

Findings include:

For Resident #13, the facility failed to prevent the negative pressure wound therapy (wound vac) tubing from lying on the floor. Resident #13 was admitted to the facility in 7/2017 with [DIAGNOSES REDACTED]. The initial wound assessment of 7/7/17 indicated that the resident’s right ankle/foot diabetic ulcer measured 3.0 centimeters (cm) in length by 3.0 cm in width by 1.0 cm in depth.

The admission Minimum Data Set assessment, with an Assessment Reference Date of 7/14/17, indicated that the resident had an intact cognitive status, required assist with hygiene, dressing, transfers, toilet use and bed mobility, was dependent with bathing, had a diabetic foot ulcer and had an infection of the foot. The 7/2017 Physician order [REDACTED].

Observation, on 7/31/17 at 12:00 P.M., 12:20 P.M., and 1:00 P.M., found Resident #13 sitting in a wheelchair in his/her bedroom. The wound vac was on the floor next to the wheelchair, and the wound vac tubing and connector were lying on the floor. During an interview, Resident #13 said that he/she disconnects the wound vac tubing, when getting dressed, to get his/her pants on. Resident #13 said he/she disconnects the tubing because the wound vac does not always fit through the leg of his/her pants. Resident #13 said that he/she reconnects the tubing after donning pants, and wishes that he/she had an alcohol pad to wipe off the ends of the connector.

Observation, on 8/1/17 at 7:30 A.M., found Resident #13 lying in bed sleeping. The wound vac was attached to the side of the bed. The tubing was lying on the floor. Observation, on 8/1/17 at 9:00 A.M., found Resident #13 sitting in a wheelchair in his/her bedroom. There was a walker with a basket in front of him/her. The wound vac was inside the basket, and the tubing was lying on the floor. Resident #13’s foot was on top of the tubing.

During an interview, on 8/3/17 at 9:30 A.M., the Assistant Director of Nurses said that the wound vac and tubing should not be on the floor. During an interview, on 8/7/17 at 8:30 A.M., the Director of Nurses said that the wound vac and tubing should not be on the floor and was an infection control concern.
